Please excuse me if I starts to sound like a sop story, I know it is my own fault.....
I have really messed up my second year about 2 months in my second year I started to feel the stress and strain of university, and hid from it, staying in bed and not doing my assignments. MY tutor allowed me to have another chance and give me an extension on 2 assignments, But these where due when 3 other where due and I fell apart with worry again staying in bed and hoping the problem would disappear.
I have yet to complete to assignments, I went to see a doctor and they diagnosed me with depression due to untreated stress, and he has given me a private sick note.
What should I do, should I do the assignments and hand them in with a Personnel Extraneous Circumstances Form, not do the assignments and hand in the P.E.C with the sick note or give up and start year 2 again.

The first thing to do is to let the uni know what has transpired. Always keep them informed. You will need to submit an ECF. together with the doctor's note.
There must be a reason for the depression. Make an appointment with the uni councillor, he/she will be very experienced and can help you work through the problems that caused the depression.
It might be worth considering repeating the year. Talk to your tutor again.
